I was a PA on the show, on set the day of the accident, a lot of folks had been at a party the night before & many of those had obviously done all nighters, the 1st AD was charged with negligence, not sure what the outcome was, but I didn’t see that 1st AD on a call sheet for several years…

What a terrible event to have been on the scene for. You would think this would have put an end to this sort of accident but that human problem is always going to crop up.
